- Los anticoagulantes orales (ACO) en monoterapia son el tratamiento de elección para la prevención de ictus asociados a fibrilación auricular (FA).

- La indicación de tratamiento anticoagulante está determinada por el riesgo de ictus, debiendo evaluarse también el riesgo hemorrágico, lo que permite identificar y corregir factores de riesgo hemorrágico modificables.

- Aproximadamente un 30% de los pacientes con FA presentan concomitantemente enfermedad coronaria.

- Los antiagregantes plaquetarios (AGP) constituyen el tratamiento de primera elección en la prevención de eventos tromboembólicos por patología coronaria.

- La combinación ACO + AGP está indicada, en pacientes con FA y alto riesgo de ictus, cuando sufren un síndrome coronario agudo (SCA) y en aquellos que presentan enfermedad coronaria estable (ECE) y se les realiza una intervención coronaria percutánea (ICP) e implantación de stent.

- La estrategia óptima de tratamiento y su duración se establece en función de la indicación (SCA o ECE+ICP/stent) y del riesgo tromboembólico y hemorrágico de cada paciente.

No se recomienda:

- Iniciar terapia antitrombótica sin evaluar riesgo tromboembólico y hemorrágico del paciente.

- Tratar con terapia combinada ACO + AGP a pacientes con FA y ECE (sin implantación de stent).

- Prolongar más de doce meses la terapia combinada ACO + AGP.

- Oral anticoagulation (OAC) as monotherapy is the treatment of choice for the prevention of stroke events associated with atrial fibrillation (AF).  

- The indication for anticoagulation therapy is determined by the risk of stroke of the patient.

- Concomitant coronary heart disease is present in approximately 30% of patients with AF.

- Antiplatelet (AP) agents are the first choice of treatment for the prevention of thromboembolic events due to coronary heart disease.

- A combination of OAC+AP is indicated for patients with AF and at high risk for stroke, suffering an acute coronary syndrome (ACS), and for those patients with stable coronary heart disease (CHD) who have undergone percutaneous coronary intervention (PCI) with stent implantation.

- The optimal treatment strategy and duration is established in line with the indication (ACS or stable CHD+PCI/stent) and the risk for thromboembolic and haemorrhagic events of each patient.

It is not recommended:

- To start antithrombotic therapy without a previous assessment of the thromboembolic and  haemorrhagic risk of the patient.

- To offer combination treatment with OAC+AP to patients with AF and stable CHD (without stenting).

- To extend combination therapy with OAC+AP beyond 12 months.
