Review of prescriptions to avoid safety issues
Prescribing reviews mainly aim to identify errors, detect potential safety issues and assess the adequacy and duration of treatments.
A medication review is mainly recommended in the following patients: the polymedicated, those who have a reduced life expectancy, the elderly, and, in particular, the frail.
Reducing or withdrawing (deprescribing) medication should be considered when, during the review process, unnecessary medication is identified or when risks outweigh the benefits.
Within the Andalusian Health Service, a periodic review of the medications of patients treated with antidiabetics, NSAIDs, benzodiazepines, antihypertensives of the renin-angiotensin system (ACEI and ARA II) is carried out in an effort to try to avoid safety issues.
